Telehealth Visits - Esperanza Health Center

TELEHEALTH VISITS

Esperanza Health Center now offers TELEHEALTH  visits with your Medical and Behavioral Health providers. It is a safe and easy way to consult with your health care provider without having to come into the office.  Telehealth is a secure telephone appointment to support patient needs. It may be done through a secure video link, or a standard phone call.

    To schedule a VIDEO TELEHEALTH APPOINTMENT with a provider, you will need to do the following:

    1. Call us to schedule a medical appointment, or request an appointment online through the Patient Portal.
    2. Make sure we have your current cell phone number on file.
    3. You will need a smartphone or phone.
    4. You will need to use Chrome, Firefox, or Safari browser
    5. Around the time of your appointment, you will receive a text from an 882-86 number. Your provider’s name will be in the text message. This message will be in English. The phone number and link will be specific to your provider.
    6. Open the link.
    7. Tap the pop ups to enable microphone and camera.
    8. Tap “Join Video Call,” and you will be connected to your provider.

    To schedule a AUDIO TELEHEALTH APPOINTMENT visit with a provider, you will need to do the following:

    1. Call us to schedule a medical appointment, or request an appointment online through the Patient Portal.
    2. Make sure we have your current phone number on file.
    3. Please be sure your phone is not on silent
    4. Near the time of your scheduled appointment, watch for an incoming call from “ESPERANZA”
    5. When you answer, your provider will already be on the call
    6. If you do not answer and try to call back, you will not be able to reach the provider directly. All return calls are answered in our call center.  Please explain to the operator that you just missed a scheduled telehealth appointment with the provider.  Our call center will attempt to reach the provider and connect you.  If the provider is unavailable to take your call, we will send a message or help you reschedule.
