Social Services
Esperanza’s Social Services department seeks to assist our patients in obtaining vital resources necessary for their overall well-being. Our Social Services staff members work with patients and their family members, offering emotional, social and spiritual support, and helping patients access other resources and assistance necessary for continuity of care. They collaborate with Esperanza’s clinical team to establish a safe and appropriate care plan to meet the needs of each patient. In addition, we seek to serve as a resource for area churches and ministries.
Social services assistance is available for, but not limited to, the following areas:
- Behavioral health services
- Substance abuse & drug addiction
- Prescription and insurance coverage
- Transportation
- Welfare and Social Security assistance
- Advance Directives and end-of-life issues
- Domestic violence and other family issues
- Food and nutrition
- Utility assistance
- Dental care
Esperanza’s Insurance Assisters also provide new and established Esperanza patients with determining their eligibility for insurance plans and programs, including Medicaid and Affordable Care Act (“ObamaCare”) insurance plans.
