Sports Participation Forms

If your child will be participating in high school sports during the coming the academic year in a high school in Pennsylvania, they will need to have a sports participation form completed by your child’s primary clinician. If your school did not provide you a form, the form can be printed using the link below.

Please bring the form with Sections 1-5 already completed to your child’s visit. The clinician will complete section 6 at the visit.
