Social Services

Esperanza’s Social Services department seeks to assist our patients in obtaining vital resources necessary for their overall well-being. Our Social Services staff members work with patients and their family members, offering emotional, social and spiritual support, and helping patients access other resources and assistance necessary for continuity of care. They collaborate with Esperanza’s clinical team to establish a safe and appropriate care plan to meet the needs of each patient. In addition, we seek to serve as a resource for area churches and ministries.

Here’s a few of the areas Social Services can provide support:

Medical:

  • Behavioral health services
  • Substance abuse & drug addiction
  • Prescription and insurance coverage
  • Domestic violence and other family issues
  • Advance Directives and end-of-life issues

Resources:

  • Food access and nutrition
  • Utility assistance
  • Transportation
  • Welfare and Social Security assistance
