Compassionate and holistic health care

Comprehensive Bilingual Healthcare

Our team of clinicians provide preventive care to help you stay healthy and also care for those who are sick and have chronic health conditions. Our adult primary care providers are bilingual in English and Spanish and include nurse practitioners, physician assistants, family physicians and internal medicine physicians.

Preventive Care

Preventive care for adults at Esperanza includes:

  • Initial comprehensive health assessment and examination and follow-up visits
  • Screening for obesity, diabetes, high blood pressure, and cholesterol
  • Gynecological care including routine Pap smears, family planning, and colposcopy
  • Confidential on-site HIV testing and Hepatitis C testing
  • Immunizations for flu, pneumonia, shingles, hepatitis A, hepatitis B and tetanus

We also provide referrals for breast (mammograms)  and colon cancer screening.

Urgent Care

We provide care for urgent health problems and acute illness, such as sickness involving fever, severe cough, fainting or dizziness and minor lacerations and injuries.

Please call 215-302-3600 if you have an urgent health need and call 911 if you have a life-threatening emergency.

Chronic Disease Management

If you have a chronic condition, such as diabetes, high blood pressure or asthma, your primary care provider will help you to develop a personal care plan.

  • Diabetes Support:  We have a diabetes team composed of a coordinator and navigators who walk alongside our patients struggling to control their diabetes to provide support, remind them of necessary screening measures, and connect them with resources.
  • Chonic Conditions: For patients with asthma, obstructive lung disease, coronary artery disease, congestive heart failure and other chronic conditions, our primary care providers partner with specialists who have extensive experience in the management of these illnesses.
  • Lifestyle Medicine Class: This class helps prevent or reverse chronic disease by teaching on nutrition through whole food plant based cooking demonstrations, exercise, and social connection.

Hospital Care

A team of Esperanza doctors directly oversees the care of our adult patients admitted to Temple University Hospital, and will be pleased to evaluate and treat you there in conjunction with Temple staff and specialists, should you require hospital care. At the time of discharge, Esperanza doctors will coordinate your discharge and arrange follow up visits with your Esperanza primary doctor and care team.
