For our patients who are uninsured, payment is on a sliding-fee scale based on household income (see details below).\n\nUninsured Persons who are HIV+ will receive care at no cost to them through a Ryan White Grant.\n

                Important Information for New Patients<\/strong><\/h4>\n

                • All new patients must begin care with a scheduled New Patient Exam appointment. A limited number of New Patient Exams become available on the first Wednesday of each month. Patients are encouraged to call for the first appointment rather than walking in.<\/li>\n \t
                • Once a New Patient Exam has been scheduled, you will be sent registration forms for completion. New patients will bring completed forms to their first appointment, and should plan to arrive 30-45 minutes prior to the scheduled appointment time to review paperwork with receptionist.<\/li>\n \t
                • All patients MUST bring their insurance card and official photo identification to the first appointment.<\/li>\n \t
                • If available, please bring a summary of your medical history, current health status, medications taken, and recent test results or referral documentation.<\/li>\n \t
                • If you do not have insurance, you may apply for assistance in the Uninsured Patient Program. You will need to meet with the Patient Access Liaison (PAL) prior to your first appointment to review your household proof of income and determine eligibility. Proof of your household income may include:\n

                  • Most recent tax return<\/li>\n \t
                  • Two recent paycheck stubs for each working family member<\/li>\n \t
                  • Letter from your employer stating your income if working under the table<\/li>\n \t
                  • SSA, SSI, or Unemployment award letters<\/li>\n<\/ul>\n<\/li>\n \t
                  • Patients of Esperanza Health Center will not be denied access to services due to inability to pay.
