As an Esperanza patient, with guidance from your primary care provider we encourage you to take an active role in your health. Preventive care for adults at Esperanza includes:<\/p>\n
We provide care for urgent health problems and acute illness, such as sickness involving fever, severe cough, fainting or dizziness and minor lacerations and injuries.<\/p>\n
If you have an urgent health problem, please call our<\/strong> Nursing staff (215-302-3600)<\/strong>; we can provide guidance and an appointment time.<\/p>\n In the event of a life-threatening emergency, always first call 911.<\/em><\/strong><\/span><\/p>\n If you have a chronic condition, such as diabetes, high blood pressure or asthma, your primary care provider will help you to develop a personal care plan.<\/p>\n A team of five Esperanza doctors directly oversees the care of our adult patients admitted to Temple University Hospital, and will be pleased to evaluate and treat you there in conjunction with Temple staff and specialists, should you require hospital care. Preventive care for adults at Esperanza includes the following:<\/p>
Esperanza provides access to same-day care for the evaluation and treatment of a sudden illness or worsening of existing illness. Examples include sickness involving fever, severe cough, shortness of breath, vomiting, severe headache, chest pain, and fainting or dizziness. Treatment of minor injuries and wounds\/lacerations is also offered on a same-day basis. By calling our nursing staff, we can give you appropriate guidance and an appointment time at one of our locations (below). In the event of a life-threatening emergency, always call 911.<\/strong><\/p>
We help our patients in the treatment and management of chronic conditions including diabetes, high blood pressure and asthma. We have a specially-trained team providing state-of-the art care for HIV disease\u2014for more information, click here (link to HIV Care page).<\/p>
In addition, bilingual community health promoters have been trained to come alongside persons in homes, churches and community centers to teach chronic illness self-management principles.
