Primary Care Physicians

Director of Quality Assurance & Population Health

Esperanza Health Center seeks a full-time, board certified Physician / Nurse Practitioner / Physician Assistant / Registered Nurse to serve as the Director for Quality Assurance and Population Health.  The Director for Quality Assurance and Population Health is responsible for the overall administration of Esperanza’s Quality Assurance Team and clinical risk management initiatives. Associate Director of Social Services and HIV Care

Esperanza Health Center seeks a full-time Associate Director of Social Services and HIV Care.  The Associate Director of Social Services and HIV Care leads the Social Services Department and HIV care team and to ensure quality of service, staff management, and operational effectiveness. Other responsibilities include: supervising the HIV care team; carrying a caseload of HIV patients; grant tracking and management; supervising the work of the Social Services Managers; and providing direct social work practice with patients. The Associate Director of Social Services and HIV Care serves as a liaison to the staff to other outside organizations. This position reports to the Director of Behavioral Health and Social Services.  This position currently includes both Social Services and HIV Care team responsibilties, but this may be subject to change; applicants qualified in Social Services responsibilities will be considered irregardless of HIV-related experience.

Clerical Assistant

Esperanza Health Center seeks a full-time Clerical Assistant.  The Clerical Assistant is responsible for registering patients’ demographics, insurance, and financial information into the EMR prior to service.  Clerical Assistants utilize insurance websites or directly contact insurance companies to verify eligibility and to confirm provider assignment.  Other responsibilities include preparing fee tickets prior to clinical visit, notating all issues concerning insurance problems or missing documentation.

Clinical Nurse - RN

Esperanza Heath Center seeks a full-time Clinical Nurse (RN) to provide triage and other direct care to adult, pediatric, and obstetric patients in the Latino and underserved communities of North Philadelphia.  This position is part of the Primary Care Team that provides holistic Christ-centered care to our patients.  Responsibilities include: assessing needs of walk-in and call-in patients, patient health education, coordinating multidisciplinary care for complex patients, providing spiritual care, training of triage staff, and support to clinical team.

This position is currently a triage role, but EHC is presently evaluating possible changes in the clinic workflow which may ultimately result in a greater focus on face to face interactions with patients in the clinic and direct support of clinicians providing primary care.

  • RN with a minimum of one year of post-graduate experience, preferably in a hospital setting

Clinical Supervisor - 5th Street

Esperanza Health Center seeks a full-time Clinical Supervisor for its 5th Street site.  This position is ½ time Clinical Supervisor, ½ time Medical Assistant.  The Clinical Supervisor at 5th Street supports the Head Nurse in leading the nursing/MA team, including: direct patient care, inventory and ordering of clinical supplies, training of staff, and involvement in clinical quality improvement measures.  Other responsibilities include: addressing the needs of walk-in and call-in patients, making arrangements for needed follow up; phlebotomy, dispensary coverage, vaccine coordination, cleaning and organizing exam rooms, and nurturing the spiritual growth of team members and patients.  This position reports to the Head Nurse at Hunting Park.

Community Health & Wellness Coordinator

Esperanza Health Center seeks a full-time Community Health & Wellness Coordinator.  The Community Health & Wellness Coordinator provides administrative support to the CH&W Department. The CH&W Coordinator is responsible for assisting with program logistics and implementation, marketing, and community programs coordination. Custodial Specialist

Esperanza Health Center seeks a part-time Custodial Specialist.  The Custodial Specialist is responsible for maintaining a clean and orderly state of the premises of Esperanza Health Center.  Additional responsibilities include maintaining environmental services equipment and supplies, performing cleaning and custodial tasks, and participating in seasonal maintenance projects such as snow removal and landscaping.  Evening and weekend availability is required.  This position is 15 hrs/ week, from 6:30pm – 9:30pm, Monday – Friday.

Director of Nursing

Esperanza Health Center seeks a full-time Director of Nursing. The Director of Nursing is responsible for the delivery of nursing and clinical support services in the organization and throughout all the EHC facilities.  Responsibilities include: development and implementation of an annual budget for the nursing department and clinical operations; maintenance of evidence based clinical policies and procedures; oversight of orientation and training of nursing department personnel and student externs;  maintenance and implementation of OSHA and employee health procedures and coordination of OSHA staff trainings; supervision of phlebotomy and POC laboratory services; coordination of QA initiatives for the nursing department; participation as Safety Officer on the Emergency Management Team; and provision of effective and efficient nursing care to patients in a multicultural, inner-city setting.

Head Nurse - RN - K&A

Esperanza Health Center seeks a full-time Head Nurse for its Kensington (K&A) clinic.  The Head Nurse is responsible for overseeing the Nursing/Clinical Support department at the K&A site.  This includes the direct supervision and leadership of staff nurses and clinical supervisors, including training, encouraging, coaching & performance management, staffing, and spiritual leadership and guidance.  Other responsibilities include: budget recommendations, oversight of laboratory compliance, vaccine protocols and management, infection control procedures, establishing goals for QA initiatives, oversight of inventory management, contributing to internal clinic initiatives such as OSHA Training and PPD influenza vaccine administration, ordering and maintenance and collaborating as needed with other departments and external agencies.  This position is primarily administrative, but fills in for nursing and clinical support staff providing direct care as needed in the case of vacancies.

HIV Care Coordinator - RN

Esperanza Health Center seeks a full time HIV Care Coordinator.   This position is responsible for coordinating and provide primary care services to patients with HIV disease, in collaboration with the center’s medical staff. Other responsibilities include: maintaining individualized documentation/care plan for HIV patients based on on-going assessment and flagging services needed; providing on-going patient education to empower patients to take an active role in managing their health; working closely with the broader HIV team to assure that patients receive needed services and referrals; assisting with HIV testing activities in the clinic, and offering spiritual care to patients in need.  This position reports to the Lead Care Coordinator.

HIV Program Administrator

Esperanza Health Center seeks a part-time or full-time HIV Program Administrator.  The HIV Program Administrator is responsible for the administration of Esperanza’s HIV Program.  This includes data collection and analysis, tracking performance measures and deliverables, recommending and implementing best practices for HIV program administration, setting department objectives and measures, and providing leadership to HIV Care Team.  The HIV Program Administrator reports to the Director of Behavioral Health and Social Services.

This position is planned as part-time, 16 hrs/week.  However, depending upon applicants’ availability, scope of interest and experience, and mutual agreement with Esperanza, this role could potentially include other responsibilities and be expanded up to full-time hours.

IT Intern

Esperanza Health Center, a faith-based bilingual primary care clinic in Philadelphia, seeks a part-time, temporary IT Intern.  The IT intern serves the IT department in a variety of end-user functions, including security access, account setup, basic technical support, issuing computers, and other IT service tickets.  This position is part-time, 5-8 hours a week, and temporary, for an estimated 5-6 months.

Medical Assistant

Esperanza Health Center, seeks a full-time Medical Assistant. The Medical Assistant will serve as a member of the Medical Assistant team providing support of the clinicians and in the provision of safe and whole person care to our patients. The Medical Assistant will be responsible for preparing adults and children for exams and assists adult practitioners and pediatricians. They will address the needs of walk-in and call-in patients; help make arrangements for needed follow up. Nurse Care Manager - CRNP / RN

Esperanza Health Center seeks a full-time Nurse Care Manager with either a CRNP or RN licensure.  The Nurse Care Manager is an integral member of the Care Management Department, focused on patients recognized as high-risk and/or high-utilizer, who require multidisciplinary care management to improve patient outcomes and reduce emergency room utilization and hospitalizations.   The Nurse Care Manager works together with primary care practitioners, community health workers and registered dietitians to deliver holistic care, develop and implement care plan strategies and patient education during in-home and in-office visits, and work with team members to provide linkages for the various health and social needs of patients.

Office Manager - Hunting Park

Esperanza Health Center seeks a full-time Office Manager for its Hunting Park clinic.  The Office Manager is responsible for ensuring patient access to care and overall satisfaction with services provided by the clerical team. The Office Manager is responsible for managing front office operations pertaining to the revenue cycle, ensuring compliance with all insurance company regulations, HRSA and Ryan White requirements, and applicable state and federal laws.  This position is responsible for supervising the Clerical team at Hunting Park, including staffing and performance management.  This position reports to the Practice Administrator.

Office Assistant

Esperanza Health Center seeks a part-time Office Assistant.  The Office Assistant position is a part time support position. The Office Assistant is is responsible to provide clerical assistance to the Human Resources department and the Finance department. The position is responsible for processing interoffice and incoming/outgoing mail at the Hunting Park site, Merchantware reports, HR bills and correspondence, hospitality requests, retreat and training arrangements, clearance and other compliance tracking, and other routine tasks. The HR/Finance Assistant performs these functions under the direct supervision of the Staff Accountant and is responsible to independently manage the daily task list efficiently.  This position is part-time, 15 hrs/week.  The schedule is 3-hour shifts M-F, mid-day or afternoon.

Esperanza Health Center seeks a full-time Referral Tracking Specialist.  The Referral Tracking Specialist is responsible to ensure that all clinician orders (consultations, tests, and labs) are completed and the results are integrated into the patient electronic health record (EHR). If the order is not completed, the Referral Tracking Specialist will take appropriate steps to follow-up, closing the loop on the referral/lab with the patient, specialist, and in the EHR, working closely with clinical teams to assure that the patients are getting the care that was requested and that there is proper documentation in the EHR.  This position reports to the Practice Administrator.

Education, Skills, and Experience

  • High school diploma or GED.  Additional relevant certificate or education, preferred.
  • 2+ years’ experience in medical setting. Must understand medical terminology. Extensive referral experience preferred
  • Strong analytical skills, with proven ability to problem-solve and prioritize tasks
  • Ability to work both independently and with larger teams
  • Ability to track projects utilizing reports and spreadsheets
  • Proven ability to document accurately and concisely in medical record.
  • Ability to communicate professionally in written and verbal forms, in Spanish and English.

Registered Dietitian - RD

Esperanza Health Center seeks a part-time Registered Dietitian (RD).  The Registered Dietitian is an integral member of the Care Management team, responsible for coordinating the implementation of nutrition counseling among the patient population, accomplishing the goals set forth for disease prevention, and overseeing nutrition education initiatives.  This includes providing individual counseling sessions, collaborating with clinicians for Diabetes care management, organizing and implementing group sessions, assisting with clinic health department staff and tracking outcomes in regards to specific grants and general department and clinic goals.  This position is part-time, 2 days/week.

Education, Skills, and Experience

  • Bachelor’s Degree in Food and Nutrition or Dietetics from an accredited college or university, required

Wellness Assistant

Esperanza Health Center seeks a part-time Wellness Assistant.  The Wellness Assistant provides customer service for patients and community members in the fitness room, as well as administrative and programmatic support for other community wellness initiatives such as FamilyFit programming, Esperanza Fresh, fitness/group exercise classes, gardening, etc. This position is critical for program promotion, linking individuals and families to available programs and providing program implementation support.  This position is part-time, 16 hrs / week.

Education, Skills, and Experience

  • High School Diploma/GED or one year customer service required.
  • Must be willing to become CPR/AED certified within 90 days of employment.
  • Bilingual, English and Spanish required.
  • Experience providing customer service and ability to demonstrate hospitable attitude to all participants, guests and staff.
  • Able to follow directions and guidelines accurately and ask for input appropriately.
  • Able to motivate, encourage and develop relationships with participants.
