The majority of our 200+ staff members live in the neighborhoods of North Philadelphia served by our health centers. Our team includes both life-long north Philadelphia residents and relative newcomers, some of whom have relocated from other parts of the U.S. and even other countries to serve here. Primary Care Physicians

Dentist

Esperanza Health Center seeks a full-time Dentist.  The Staff Dentist is responsible for providing clinical dental services and preventive dental care in a community health center setting.  The Staff Dentist supervises the dental assistants and dental hygienists of the community health center in the absence of the Lead Dentist.

Education, Skills, and Experience

Assistant Office Manager

Esperanza Health Center seeks a full-time Assistant Office Manager.  Under the direct supervision of the Practice Administrator, the Assistant Office Manager is responsible for providing direct support to the Office Managers to ensure quality operations within the Clerical department. The Assistant Office Manager is responsible for aspects of clerical scheduling, referrals, high risk referral tracking, and PAL support. The Assistant Office Manager trains the new hires and provides coaching as needed. The Assistant Office Manager is responsible for preparing reports as requested. Behavioral Health Consultant - LCSW / Psy.D

Cafe Team Member

Esperanza Health Center seeks a part-time Café Team Member.  The Café Team Member is responsible for providing healthy food options for the patients, staff, and the general public connected to the clinic community.  The Café Team Member performs the food preparation, customer service, and kitchen sanitation tasks.  Additional responsibilities include driving and purchasing goods and supplies; stocking café supplies, operating the cash register, monitoring the functioning of equipment; and opening and closing the café for operations.  This position is part-time, 23.5 hours per week.  Shifts are M-F between 8:00am and 2:30pm

Clerical Assistant

Esperanza Health Center seeks a full-time Clerical Assistant.  The Clerical Assistant is responsible for registering patients’ demographics, insurance, and financial information into the EMR prior to service.  Clerical Assistants utilize insurance websites or directly contact insurance companies to verify eligibility and to confirm provider assignment.  Other responsibilities include preparing fee tickets prior to clinical visit, notating all issues concerning insurance problems or missing documentation.

Director of Nursing

Esperanza Health Center seeks a full-time Director of Nursing. The Director of Nursing is responsible for the delivery of nursing and clinical support services in the organization and throughout all the EHC facilities.  Responsibilities include: development and implementation of an annual budget for the nursing department and clinical operations; maintenance of evidence based clinical policies and procedures; oversight of orientation and training of nursing department personnel and student externs;  maintenance and implementation of OSHA and employee health procedures and coordination of OSHA staff trainings; supervision of phlebotomy and POC laboratory services; coordination of QA initiatives for the nursing department; participation as Safety Officer on the Emergency Management Team; and provision of effective and efficient nursing care to patients in a multicultural, inner-city setting.

Director of HIV Care and Social Services

Esperanza Health Center seeks a full-time Director of HIV Care and Social Services.  The Director of HIV Care and Social Services leads the HIV Team and Social Services Department to ensure quality of service, staff management, and operational effectiveness. Other responsibilities include: supervising the HIV care team; carrying a caseload of HIV patients; grant tracking and management; supervising the work of the Social Services Managers; and providing direct social work practice with patients. The Director of HIV Care and Social Services serves as a liaison to the staff to other outside organizations. This position reports to the Director of Behavioral Health and Social Services.

HIV Care Coordinator - RN

Esperanza Health Center seeks a full time HIV Care Coordinator.   This position is responsible for coordinating and provide primary care services to patients with HIV disease, in collaboration with the center’s medical staff. Other responsibilities include: maintaining individualized documentation/care plan for HIV patients based on on-going assessment and flagging services needed; providing on-going patient education to empower patients to take an active role in managing their health; working closely with the broader HIV team to assure that patients receive needed services and referrals; assisting with HIV testing activities in the clinic, and offering spiritual care to patients in need.  This position reports to the Lead Care Coordinator.

IT Intern

Esperanza Health Center seeks a full-time Licensed Practical Nurse (LPN).  The LPN serves the needs of the clinical team by addressing the needs of walk-in and call-in patients.  Other responsibilities include: gathering and documenting patient’s chief complaint and signs and symptoms of illness; participating in patient health education; performing medication reconciliation and patient teaching around medication adherence; assisting in coordinating care of complex or acutely ill patients; providing spiritual care to patients; administering vaccines and injectable medications; and assisting with other clinical procedures.  This position reports to the Director of Nursing.

Medical Assistant

Esperanza Health Center, seeks a full-time Medical Assistant. The Medical Assistant will serve as a member of the Medical Assistant team providing support of the clinicians and in the provision of safe and whole person care to our patients. The Medical Assistant will be responsible for preparing adults and children for exams and assists adult practitioners and pediatricians. Nurse Care Manager - CRNP / RN

Esperanza Health Center seeks a full-time Nurse Care Manager with either a CRNP or RN licensure.  The Nurse Care Manager is an integral member of the Care Management Department, focused on patients recognized as high-risk and/or high-utilizer, who require multidisciplinary care management to improve patient outcomes and reduce emergency room utilization and hospitalizations.   The Nurse Care Manager works together with primary care practitioners, community health workers and registered dietitians to deliver holistic care, develop and implement care plan strategies and patient education during in-home and in-office visits, and work with team members to provide linkages for the various health and social needs of patients.

  • CRNP and/or Registered Nurse license required; MSN preferred

Esperanza Health Center seeks a part-time Registered Dietitian (RD).  The Registered Dietitian is an integral member of the Care Management team, responsible for coordinating the implementation of nutrition counseling among the patient population, accomplishing the goals set forth for disease prevention, and overseeing nutrition education initiatives.  This includes providing individual counseling sessions, collaborating with clinicians for Diabetes care management, organizing and implementing group sessions, assisting with clinic health department staff and tracking outcomes in regards to specific grants and general department and clinic goals.  This position is part-time, 2 days/week.

Education, Skills, and Experience

  • Bachelor’s Degree in Food and Nutrition or Dietetics from an accredited college or university, required
  • RD licensure, required; CDE, preferred
  • Bilingual in Spanish and English, required
  • Strong cross cultural skills and sensitivity
  • Ability to integrate faith in practice
  • Able to handle many details and multi-task well
