Dentist

Esperanza Health Center seeks a full-time Dentist.  The Staff Dentist is responsible for providing clinical dental services and preventive dental care in a community health center setting.  The Staff Dentist supervises the dental assistants and dental hygienists of the community health center in the absence of the Lead Dentist.

OB/GYN Coordinator

Esperanza Health Center seeks a full-time OB/GYN Coordinator.  The OB/GYN Coordinator will be an integral part of Esperanza Health Center’s obstetrics and gynecological team.  The OB/GYN coordinator will manage the Centering Pregnancy program and have administrative and clinical responsibilities for the department.   Responsibilities include recruiting patients for the Centering Pregnancy program and performing follow-up, organizing and facilitating patient groups, serving as an Esperanza representative to outside organizations in matters related to Centering Pregnancy, capturing relevant data and tracking various outcome measures, providing walk-in pregnancy testing and care coordination, lactation support and various other responsibilities as assigned within the OB/GYN department.

Addictions Counselor

Esperanza Health Center, seeks a full-time Addictions Counselor. The Addictions Counselor provides drug and alcohol consultation, screening, brief intervention and referral services for patients with substance use issues. The Addictions Counselor collaborates with patients, and as appropriate, their family members and/or support system and the medical treatment team to develop a plan to address substance use issues, including providing recommendations, referrals and coordination of care. He/she works with insurance companies to obtain necessary authorizations and with pharmacies in order to ensure adequate supply of medication needed for treatment. The Addictions Counselor develops and leads support groups for patients and/or family members. The Addictions Counselor, as needed, will provide integrated behavioral health services to patients who may not have a substance abuse diagnosis.

Education, Skills, and Experience

  • Master’s in Social Work (LSW or LCSW) preferred, other licensures will be considered (LPC, LMFT, PsyD)
  • Certified Addictions Counselor credentials required within 6 months of employment
  • At least 1 year work experience in a medical or behavioral health setting, preferably with experience in addictions counseling/treatment.

Clerical Assistant

Dental Assistant

Esperanza Health Center seeks a full-time Dental Assistant.  The Dental Assistant is responsible for assisting the Dentists and Dental Hygienists in the direct provision of dental services to patients of the center. Electronic Medical Records Support Specialist

Esperanza Health Center seeks a full-time Electronic Medical Records (EMR) Support Specialist.  The EMR Support Specialist plays a key role in providing technical expertise and direct support and ongoing training for staff using Esperanza’s GE Centricity EMR.  By working closely with the clinical staff for training and support, the EMR Support Specialist is actively involved in identifying and implementing enhancements and improvements to workflows and system features.  Primary responsibilities include directing IT desk support, conducting EMR training sessions and follow-up, performance and process improvement, content development for workflow analysis, and maintaining EMR permissions and user lists.

Group Fitness Instructor

Esperanza Health Center, a faith-based primary health care clinic in Philadelphia, seeks a part-time Group Fitness Instructor.  The Group Fitness Instructor will provide group class fitness instruction for patients and community members with a special focus on participants who are new to exercise, have been away from exercise for a period of time, or who otherwise desire to participate in low impact, low to moderate intensity physical exercise in a group setting. Other areas of interest include classes for children and youth fitness, teen fitness, senior fitness, family fitness, martial arts, dance, kickboxing, yoga, Pilates or other specialty areas. The Group Fitness Instructor is responsible for preparing and leading group fitness classes according to a schedule, intensity level and format agreed upon by supervisor. This includes demonstrating and leading the groups in a variety of movements done safely to provide participants with the opportunity to increase cardiovascular endurance, gain muscular strength, increase flexibility, or meet other specified fitness goals.  The Group Fitness Instructor may also be responsible for registering new participants and processing payments when necessary.  The Group Fitness Instructor may also be asked to substitute for other EHC group exercise classes. Hours will depend on your certifications and our program needs.

Education, Skills, and Experience

  • Current group exercise certification required such as Zumba Gold, Kickboxing, Exercise for Special Populations or other certification from well-known certifying body (ACE, AFFA, ISSA strongly preferred.
  • Previous experience leading group exercise classes required.
  • Able to design and lead a group exercise class that will give participants with diverse fitness levels, needs and goals a sense of accomplishment and success.
  • Strong interpersonal and cross-cultural skills

Medical Assistant

Esperanza Health Center, seeks a full-time Medical Assistant. The Medical Assistant will serve as a member of the Medical Assistant team providing support of the clinicians and in the provision of safe and whole person care to our patients. The Medical Assistant will be responsible for preparing adults and children for exams and assists adult practitioners and pediatricians. They will address the needs of walk-in and call-in patients; help make arrangements for needed follow up. Outreach Specialist

Esperanza Health Center seeks a full-time Outreach Specialist.  The Outreach Specialist will be an integral member of the care management outreach team, focused on Esperanza Health Center patients recognized as high-risk and/or high-utilizer, who require multidisciplinary case management to improve patient outcomes and reduce emergency room utilization and hospitalizations. Together with the primary care practitioner and the Nurse Care Manager, the Outreach Specialist will assist with care plan implementation and patient education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of patients. This is a full-time, temporary position approved through December 31st, 2018.

Security and Front Entrance Coordinator

Esperanza Health Center seeks a part-time Security and Front Desk Coordinator.  Under the direct supervision of the Operations Director, this position is responsible for providing customer service at the HP front entrance, security services throughout the HP facility, and operational support services.  Other responsibilities include: welcoming patients and guests and providing active assistance in directing around the facility, watching and responding to issues on security monitors and general facility surveillance, coordinate shipping & receiving functions for HP site, and other operations projects such as supplies ordering and maintenance coordination.  This position is part-time, 20-24 hrs/week.  Some evenings and weekends required.
